Important Notice for Connec-to-Talk Employees

This notice is to inform current and former employees of Connec-to-Talk LLC that Form 1095-C, Employer-Provided Health Insurance Offer and Coverage, is available upon request for tax year 2024 and forward.

What is Form 1095-C?

Form 1095-C is a tax form that large employers like Connec-to-Talk are required to provide to full-time employees. It includes information about:

  • Whether health insurance coverage was offered to you
  • Whether the coverage offered met federal requirements under the Affordable Care Act (ACA)
  • Details about your health coverage for the tax year

Why This Change?

Due to recent changes in federal law (the Paperwork Burden Reduction Act), employers are no longer required to automatically mail Form 1095-C to all employees. Instead, we provide this notice to inform you that the form is available if you need it.

How to Request Your Form 1095-C

In Your Request, Please Include:

  • Your full name
  • Employee ID or last 4 digits of your Social Security Number
  • The tax year you need (e.g., 2024)

Response Timeline

Requests can be made at any time. We will provide your form within 30 days of your request or by January 31st, whichever comes first.

Do I Need This Form to File My Taxes?

Form 1095-C is not required to file your federal income tax return. However, you may need the information on this form if you:

  • Received advance premium tax credits through a Health Insurance Marketplace
  • Plan to claim the premium tax credit on your tax return
  • Keep this form with your tax records for future reference.
